Health Maintenance Organization (HMO)
HMO plans focus on care within a specific network of providers, covering out-of-network services only in emergencies, such as urgent ER visits. Typically, you must live or work in the HMO’s service area to qualify for coverage.
Due to these network limitations, HMO insurance plans might restrict your options for detox or rehab facilities outside your immediate area. However, it’s worth exploring whether emergency situations, like the need for urgent detox due to overdose or severe relapse risk, might allow for coverage at an out-of-state facility.
Point of Service (POS)

However, for out-of-network treatment, you may need a referral from your primary care physician to ensure partial or full coverage of the costs. EPO insurance plans limit coverage to in-network providers, but unlike HMOs, they don’t require you to select a primary care provider. Additionally, EPO plans typically allow you to see specialists, including those at detox and rehab centers, without needing a referral. Emergency care outside the network is always covered under EPO policies, making the approval process for treatment more straightforward.
