Surest Insurance Plan Options
Surest is a UnitedHealthcare company and an ACA-compliant employer-sponsored health plan. Only employers with 51 or more employees can access its plans, which are fully insured (and self-funded). Your insurance plan may cover addiction treatment at a rehab or detox that accepts Surest, but it all depends on the policy your employer invested in.
Addiction Treatment Options
Your addiction treatment options range from residential inpatient to outpatient care. Surest may cover some or all of the costs of treatment depending on your plan.
Surest for Medical Detox
Medical detox allows you to stop using drugs or alcohol in a safe environment. Medical detox is done on an inpatient basis. Medications used in medical detox vary depending on the types of substances used. Surest may cover 100% of medical detox. Solution Based Treatment offers alcohol detox, benzodiazepine detox and opioid detox programs that may be covered up to 100%.
Surest Coverage for Residential Treatment
Residential treatment options require you to stay overnight at your treatment facility for 30, 60, or 90 days. Rehabs that take Surest focus on providing you with around-the-clock care to overcome substance use disorder.
Surest Coverage for Outpatient Treatment
Outpatient treatment is normally covered by all insurance providers, but the exact coverage will depend on your plan. You may need to pay deductibles, co-payments, or meet out-of-pocket maximums before your provider will pay, particularly if you’re interested in using an out-of-network provider.
Intensive Outpatient Programs (IOP)
Intensive outpatient programs (IOP) take place several times a week for a few hours at a time. These programs are intensive but allow you to go home afterward, so you can maintain outside responsibilities. Surest may cover IOP programs; our Admissions Department can verify your benefits.
Partial Hospitalization Programs (PHP)
Partial hospitalization programs (PHP) take place in an inpatient facility or hospital during the day and offer consistent medical care throughout your stay. Patients in these programs return home at night but spend most, if not all, days of the week in the hospital. Surest may cover partial hospitalization programs based on your specific coverage in your schedule of benefits.
Outpatient Treatment
Outpatient treatment includes activities such as individual therapy, group therapy, medication management, and others. Surest may provide coverage for these services to remain in compliance with the American Care Act.

How is a Surest HMO different from a PPO?
A rehab program Surest insurance takes through an HMO will always be in-network. HMOs tend to be more affordable because they work with a specific network of providers who are preapproved. PPO plans allow you more flexibility and some coverage for out-of-network providers.
Can you get a single case agreement with Surest?
You may be able to get a single case agreement with Surest, which will allow you to continue care with a long-term provider who is out of network with your current insurance policy.
